NWFW Furniture Jury Application Form
Instructions:
1. Fill in the form completely.
2. DOUBLE CHECK for accuracy, once you hit the submit button, you cannot make changes.
3.(optional) Print form for your records.
4. Submit the form. (Click the SUBMIT FORM button at the bottom of this page).
5. Wait for the confirmation email or phone call from the gallery.
IMPORTANT: Please do not bring in furniture to the gallery unless you have contacted us prior to filling out this form.
Please allow time for the gallery to process this form (minimum of one week before bringing work in to the gallery preferred).
This form will not be processed on weekends or Holidays.
For multiple furniture items, fill out and submit each form individually; with the exception of multiples of the exact same item.
Pieces with different prices or wood types are considered different items.
Submitting this form does not guarantee placement of item in gallery, please confirm with the gallery before you bring in work for the jury.
